Indications for use
Folate deficiency and megaloblastic anemia, colon cancer, methotrexate overdose.
Leukocast is indicated after therapy with high doses of methotrexate for osteosarcoma.
Clinical pharmacology
Calcium folinate is the reduced form of folic acid and is used as an antidote to drugs that act as folic acid antagonists.
Folic acid antagonists, such as methotrexate, inhibit dihydrofolate reductase and thereby prevent the formation of tetrahydrofolate from folic acid, which is a cofactor for the transfer of one-carbon residues in nucleic acid biosynthesis. As a result, the synthesis of nucleic acids and cell division are blocked. Calcium folinate, unlike folic acid, does not require reduction by dihydrofolate reductase to convert into tetrahydrofolate, which allows its use to restore the impaired process of biosynthesis of DNA, RNA and proteins. The protective effect of calcium folinate occurs only in relation to healthy cells. Due to these properties of calcium, folinate prevents the toxic effect of methotrexate on the cells of the bone marrow and gastrointestinal tract, but does not significantly affect the already developed nephrotoxic effect of methotrexate.
The drug helps to compensate for the deficiency of folic acid in the body.
Calcium folinate may also enhance the antitumor effect of fluorouracil. When these two drugs interact, a stable complex of components is formed, containing thymidylate synthetase, which inhibits and suppresses DNA synthesis.
The onset of action of the drug with intramuscular administration is after 10-20 minutes, with intravenous administration — in less than 5 minutes. The duration of action is about 3-6 hours, regardless of the route of administration.

Contraindications
Hypersensitivity, pernicious and other megaloblastic anemia due to vitamin B12 deficiency (since treatment may improve hematological parameters, while neurological manifestations continue to progress).

Leucovorin calcium indications for use
- Intoxication with folic acid antagonists (methotrexate, trimethoprim, pyrimethamine);
- megaloblastic anemia due to folic acid deficiency with the ineffectiveness of oral therapy with folic acid preparations (including malabsorption syndrome, malnutrition, congenital deficiency of dihydrofolate reductase);
- colon cancer, including rectum (as an adjuvant therapy).
